I have read that cops (and coroners) can tell when a fresh batch of un-cut (or nearly so) heroin hits the streets...junkies drop like flies.
As heroin supplies age/drop/get tight, sellers cut the product to stretch sales and users up the dose to maintain a preferred high on a watered down product. When fresh, not as adulterated, product hits the street and users go with dose they have become accustomed to, blam,, system shut down.
Now I don't recall whether I read this in the NY Times or a Mickey Spillane novel, but it does make sense that an illegal product sold and re-sold by lowlifes may accumulate additional risk by dint thereof. (is that sentence legal?)
This "added risk factor" should be somewhat knowable. The Netherlands offers clean dope (and clean needles) to its citizens. Are Amsterdam junkies in better shape than New York junkies?
Until we can come up with the money and will to fund basic, affordable health care for working stiffs and their families, I can't get too fired up about pushing for the funds necessary to go Dutch with junkies. But, legalizing the product would probably make both the product and the city safer.

There are lots of things that are bad ideas. Like having a banjo in a church band.
The disconnect I see is in the impulse to convert bad ideas into crimes, and that becomes a very deep question.

A drug addict does not need violence.
